California Occupational Therapy Identification

Individual Identification

Occupational therapists and occupational therapy assistants shall wear a name tag while working, in at least 18-point font that provides the first and last name and license type.  However, the name tag requirement shall not apply to occupational therapists or occupational therapy assistants who prominently display a copy of their license in the practice area or office where he or she works.

Doctoral Degree

A licensee who has earned a doctoral degree in occupational therapy (OTD) or, after adoption of the regulations described in subdivision (e), a doctoral degree in a related area of practice or study may do the following:

  • In a written communication, use the initials OTD, DrPH, PhD, or EdD, as applicable, following the licensee’s name.
  • In a written communication, use the title “Doctor” or the abbreviation “Dr.” preceding the licensee’s name, if the licensee’s name is immediately followed by an unabbreviated specification of the applicable doctoral degree held by the licensee.
  • In a spoken communication while engaged in the practice of occupational therapy, use the title “Doctor” preceding the licensee’s name, if the licensee specifies that he or she is an occupational therapy practitioner.

A doctoral degree shall be granted by an institution and program accredited by the Western Association of Schools and Colleges, the Accreditation Council on Occupational Therapy Education, or by an accrediting agency recognized by the National Commission on Accrediting or the United States Department of Education that the board determines is equivalent to the Western Association of Schools and Colleges.

Notice of Information Changes

Each person licensed or issued a limited permit by the board, shall report to the board every change of residence address within thirty (30) days after the change, giving both the old and new addresses. In addition to the residence address, the person may provide the board with an alternate address of record. If an alternate address is the person's address of record, he or she may request, in writing, that the residence address not be disclosed to the public.

Notice to Patients

Occupational therapists and occupational therapy assistants shall provide to the patients or clients at the time of initial evaluation in at least 24-point font and in the format or display in at least 24-point font in an area visible to patients or clients, written notice specifying that the licensee is regulated by the California Board of Occupational Therapy, and provide the following personal information: licensee’s first and last name; license type; and highest level of earned academic degree related to the provision of occupational therapy services.

This information shall also be displayed on any Internet website directly controlled or administered by the occupational therapist or occupational therapy assistant or his or her office personnel. 

This rule does not apply to occupational therapists or occupational therapy assistants who work in the following types of facilities:

  • General acute care hospital;
  • Acute psychiatric hospital;
  • Skilled nursing facility;
  • Intermediate care facility;
  • Intermediate care facility/developmentally disabled-habilitative;
  • Special hospital;
  • Intermediate care facility/developmentally disabled;
  • Intermediate care facility/developmentally disabled-nursing;
  • Congregate living health facility;
  • Correctional treatment center;
  • Nursing facility;
  • Intermediate care facility/developmentally disabled-continuous nursing; or
  • Hospice facility.
