Application for a license for PT and PTA


Renewal of a license

$30 per year

Restoration of a license other than from inactive status

$50 plus payment of all lapsed renewal fees, but not to exceed $200

Restoration of a license from inactive status is the current renewal fee


Issuance of a duplicate license


Issuance of a replacement license for a license that has been lost or destroyed


Issuance of a license with a change of name or address


Certification of a licensee's record


Scoring of an examination authorized by the Division reviewed and verified

$20 plus any fees charged by the designated testing service

Wall certificate showing licensure

Actual cost of producing the certificate
