Individual Identification
Unprofessional Conduct: Using the word "doctor" in offering to perform professional services without also indicating the discipline in which the licensee holds a doctorate.
Licensees shall be identified as a physical therapist or physical therapist assistant in a legible manner to individuals that come in contact with them during the provisions of physical therapy services. The appropriate credentials, Physical Therapist or Physical Therapist Assistant, or the appropriate abbreviations, PT or PTA, shall be displayed on their person via proper identification.
Display of License
There shall at all times be prominently displayed in the place of business of each PT/PTA a sign containing the name, mailing address, and telephone number of the Board.
Notice of Information Changes
Name change. Any licensee whose name is changed by marriage or court order shall surrender his/her license, provide proof of name change and apply for a replacement license within sixty (60) days.
