Outpatient Therapy Center Enrollment

An outpatient therapy center must be certified by the Alaska Division of Health Care Services, Health Facilities Licensing and Certification Unit.

Each member of an outpatient therapy center must enroll individually with Alaska Medical Assistance.

The following providers may enroll as part of an outpatient therapy center:

    • Audiologist
    • Occupational Therapist
    • Physical Therapist
    • Speech-Language Pathologist
    • Temporarily Licensed Speech-Language Pathologist
    • Occupational Therapy Assistant
    • Physical Therapy Assistant
    • Speech-Language Pathology Assistant

Claims for services provided as part of an outpatient therapy center must be submitted with the therapy center as the billing provider and the individual as the rendering provider.  Claims for services provided outside the therapy center are subject to the individuals enrollment requirements.

Before an OTA, PTA, or SLPA enrolls with a therapy center, at least on therapist/pathologist of the same discipline must be enrolled as part of the same therapy center.
