Alabama PT Temporary License

Who is eligible for a temporary license?

Graduates who have not yet taken the exam.  Each applicant for licensure as a PT or PTA must have completed a CAPTE approved program of physical therapy education appropriate for preparation as a PT or PTA.

What is required for the application?

Completed Application with Application ($100) Fee paid.

2 Character Reference Forms.

Official School Transcript.

Jurisprudence requirement met.

Passport Style Photo.

What type of supervision is required for a temporary license holder?

PTs practicing under a temporary license must be under the direct, on-site supervision of a licensed PT.

PTAs practicing under a temporary license must be under the direct, on-site supervision of a licensed PT or a licensed PTA, with the approval of the supervising PT.

How long is the license valid for?

The temporary license shall be valid only until the first examination is scored and the Board determines whether a license shall be issued.

How should the temporary license holder be designated?

The name tag should indicate that the provider has a temporary license.
