What type of supervision is required for a limited license holder?

    • Any change or addition of a supervising occupational therapist requires the filing of a new Direct Supervision Agreement for a Limited Permit form by the supervisor.
    • The supervisor shall submit the Direct Supervision Agreement for a Limited Permit form within seven (7) days of any change or addition of a supervising occupational therapist.
    • The supervising occupational therapist shall co-sign all patient records documenting patient treatment and progress.
